If your child requires medication or an EpiPen at school, the following guidelines need to be followed:

• All medications and EpiPen's must be in date and in the original packaging with the Pharmacist label stating the child’s name and dosage (this includes non-prescription medication, eg paracetamol).
• A Parent/Guardian DoE Authorisation form needs to be completed for all medications that are to be administered at school.
• Medical Practitioner/Pharmacist DoE Authorisation forms needs to be completed for ongoing prescription medication and EpiPen’s.
• Asthma, epilepsy and anaphylaxis action plans need to be updated every year by your doctor and provided to the school office.
